The partners working on a new project to help create pathways for physical activity in health and care systems are seeking members for five Communities of Practice (CoP) that are currently being set up.
The project, which has been created so that a physical activity for health pathways framework and toolkit can be co-designed, is being led by Move Consulting and the Active Partnerships National Organisation. It has been funded by Sport England.
It is hoped that the communities of practice will attract applications from people with experience of working for a whole range of organisations including local voluntary sector bodies, public health teams, leisure operators and community activity providers, primary care and other health networks, and Active Partnerships from across the nationwide network.
The work involves the identification of tools and solutions to create local physical activity for health pathways. These pathways aim to support adults who are living with or at risk of developing long-term health conditions by providing access to behavioural change support and structured opportunities to develop and maintain a more active lifestyle.
Once the Communities of Practice are set up, the members will help to create evidence-based solutions and guidance that reflects best practice from the field, as well as being practical to implement.
A CoP is defined as a group of individuals who share a common interest or profession, coming together to learn from one another, solve problems, and develop expertise in their field through regular interaction and collaboration. In this case the role of members will be to:
- Advocate for the project and champion the role of physical activity pathways.
- Understand current practice, guidance, resources, and tools being utilised.
- Identify existing challenges faced in the system (local and national)
- Engage with their respective networks to test concepts, resources or tools.
- Share learning and insight to support the co-development of solutions.
The five CoPs will cover the key aspects of the pathway which are recruitment, triage, behaviour change, opportunities and exit routes.
It is hoped that people from organisations who are actively engaged in the co-ordination and delivery of pathways in their area, are well connected with health and /or physical activity partners, and open to collaboration with the capacity to test or review and develop solutions will apply for this opportunity.
